Walmart has iPhone 12 64GB for $2.75/month. Requires 36 months, 0% APR installment agreement, qualifying credit and qualifying service plan. New and existing customers. Activation fee apply. Shipping is free.
You are locked at $60 plan/month and cannot change lower than $50/month plan afterward. They placed this new rule recently and no room for a negotiation from my experience. //@Anonymous: Boost has it for $200, cheapest plan is $15 5GB

Yeah, it seems like a bit of a scam, right? Even though the phone's only $2 a month, you end up paying way more for the service over three years. You're better off just buying an unlocked phone upfront and finding a cheaper service, like Mint Mobile
